The first visit with our patient involves a thorough evaluation, data collection, diagnosis, and the development of a plan based on the diagnosis. The plans will vary based on the unique situation of each patient. At the end of the first visit, our patients will receive a detailed plan explaining their options and the sequence of treatments. We will also discuss our plans with the referring provider and coordinate the treatment accordingly.
Some important information to provide when visiting Green Bay Advanced Oral Surgery & Implant Center includes:
- Valid ID
- Insurance information, if any
- A detailed medication list, including dosage, frequency of taking medications, and herbal supplements.
- Any relevant information from your dentist or provider, including images (X-rays). Your provider often forwards these images to our practice, but please ensure they are forwarded.
- If a patient is under the age of 18, we require a legal guardian to be present at each visit.
- Please inform us if you have any other medical concerns or require any special arrangements.
